The single most common mistake in VOD review is starting from the round you lost. By the time a round is lost the decision that lost it is usually two or three rounds behind you, sitting in an economy call or a map read nobody flagged because it did not immediately hurt.
Here is the method I use with both of my rosters. Watch the whole half once at speed with no pausing and write down only the rounds where the scoreline moved against expectation. Not the rounds you lost. The rounds you lost that you expected to win, and the rounds you won that you expected to lose. Those two lists are where the information is.
Then, for each one, go back three rounds and watch forwards. You are looking for the moment the plan stopped being the plan. Nine times in ten it is not a mechanical error, it is a piece of information that arrived and did not get acted on, or a piece of information that never arrived because nobody asked for it.
Ninety minutes, structured like that, will produce more usable notes than four hours of watching every round and pausing on every death. It also survives contact with a team that has jobs, which the four-hour version does not. The four-hour version is how you get a roster that stops turning up.
